HANDOFF/REPORT SHEET

An important skill that you will develop and critique over time is how to set up your report/handoff sheet. Some nurses like to use the premade/preformatted ones. These are great if you’re able to ignore the erroneous spaces or blocks you don’t need – it makes my brain hurt. I am going to show you an example of a premade one and an example of how I set up my handoff sheet. For my sheet, I use a blank piece of paper and fold it in ½ lengthwise – so you have the 2 fronts and the 2 insides (when you open it). I used the front with the below format and then on the inside I listed meds/times, hourly I&O’s, any changes made during rounds, etc. There is an example of this and pictures of a sample report sheet below. Obviously make any changes you’d like, I’m simply providing a starting place.
